Do you have any other health conditions? This does not include your learning disability.
What type of housing do you currently live in? For example, a family home, a home you own, a home rented from a landlord, council, housing association or charity, shared housing, care home, inpatient in hospital.
If where you are currently living is not your usual home, please describe where you usually live and the reason for not currently living here.
Please list anyone else you live with. For example, family, partner, friends, other residents, patients in hospital.
Please tell us about any care and support that you receive in your home. Please give examples of what they help with and how often they visit.
