CASS Practice Evaluation (old)

Overview

This is the new CASS Practice Evaluation online tool.

Please complete the following form for each referral you have been allocated to audit.  Simply restart the survey each time, and make sure you go all the way through the survey for each referral and submit the form.

You will be asked to complete 6 evaluations for two months running, and in the third month the results will be analysed and a learning discussion will take place.
