CASS Practice Evaluations (new 2022)

Overview

This is the new CASS Practice Evaluation online tool.

Please complete the following form for each PER ID that has been allocated to you to audit. For each audit you will need to review the RFSF/ contact and/or Referral form.

For each referral make sure you go all the way through the survey and press submit, then simply restart the survey each time for the next referral allocated to you.
