Overview
Falls and fractures prevention is a key priority for Birmingham Public Health. It is important to capture the views of our citizens, carers and family members that have been affected by a fall or fracture. This questionnaire is part of a wider consultation and understanding of how we can improve your experience of the services you used/are using. The questionnaire below will enable us to gain your views about what is happening at a local level and help us develop effective services to meet the requirements of those most at need. It will also provide us with the opportunity to understand your experiences of the wide variety of organisations and groups across the city.
Please help us by filling in this questionnaire if you are aged over 60 years or over and have had a fall within the last 3 years.
