Support Following Discharge from Hospital or Rehabilitation Bed - Your Views
Overview
Spending time in hospital or in a rehabilitation unit can be a challenging and stressful experience for citizens. The priority is to get citizens home as quickly as possible with the right support in the community, as required, in order to minimize the risks of loss of condition and independence that are associated with spending too long in bed-based care. There are a number of services in place to provide initial support for citizens returning home to ensure a safe transition.
Why your views matter
Now more than ever, we are keen to look at how we can improve the support offered following discharge, to ensure that citizens arrive safely at home and are able to remain there. During Covid a number of changes were made to the support available; increasing the offer. Now we must take the opportunity to review how the landscape has changed in order to ensure that we are commissioning services that are fit for purpose.
