Learning Disability Housing - Citizens Questionnaire (Physical and Mental Health)
Overview
The Public Health Evidence Team at Birmingham City Council are writing a report on the housing needs of people with learning disabilities in Birmingham.
We would like to hear from people with learning disabilities about how your housing affects your health.
If you would like to, please ask someone you trust to help you answer these questions.
We will use your answers from this questionnaire to help us write our report. Any answers we use in our report will be anonymous. This means that nobody will be able to know it was you that gave the answer.
Birmingham City Council will follow their Processing Data Rules (see these rules here Privacy statement) to store any information you share for this project.
