About the Person You Support

Has the person you support been diagnosed with a learning disability?

At what age were they diagnosed? If they have not yet been diagnosed as having a learning disability, please tell us where in the process of assessment or diagnosis they are.
Aside from having a learning disability, does the person you support have any health conditions?
What type of housing do they currently live in? E.g., a family home, a home they own, a home rented from a landlord, council, housing association or charity, shared housing, care home, inpatient in hospital.
If where the person you support is currently living is not their usual home, please describe where they usually live and the reason for not currently living there.
Please list anyone else the person you support lives with. E.g., family members, friends, partner, other residents, patients in hospitals.
Please describe any care or support they receive in their home. Please give examples of what they help with, and how often they visit.
