Welcome to the All Staff Health Check Survey!

Thank you for taking the time to complete the Health Check. The survey has eight sections covering:

  1. Organisational Culture
  2. Management Support and Approach
  3. Team Communications and Support
  4. Learning and Work Resources
  5. Working Remotely
  6. Job Satisfaction
  7. Wellbeing
  8. Staff Information

You have the opportunity to give us written suggestions in each section, but you do not need to.  The survey will take between 15 and 20 minutes to complete depending on how much you would like to say!

Who it is for?

The Health Check survey is for all staff. We recognise that we all work in different ways and have different pressures in our roles and want to hear the views of all our staff in their varied roles.

For frontline practitioners, there will be an additional seven questions during the survey relating specifically to your roles.

Confidentiality

Towards the end of the survey you have the option to enter your email address if you would like a copy of your response.  This is optional and you do not need to enter your email, you can just leave it blank.  If you do enter your email address this is not received as part of your survey response, so it remains confidential.   

This online survey tool automatically collects your IP address, but this is not used and will be deleted when the responses are downloaded. Your responses will be anonymous, and will only be analysed by staff in the Practice Hub.

Why your views matter

I appreciate it can be difficult to find 20 minutes in your busy days, but please do take this chance to let us know what you think and tell us about your experiences.  We are a learning organisation that needs to hear from all staff in different roles to make a difference!
